Intro Population health planning is based on the social model of health, aiming to improve the health of entire populations and reduce inequities, recognising that environmental, economic, social, cultural and behavioural factors contribute to health and wellbeing. Identifying causes of poor health is critical, as addressing these causes is the main method of improving population health outcomes. As the range of possible causative factors is vast, and are often interdependent, population health planning cannot be the responsibility of only a single sector, local government or organisation. Effective planning utilises partnerships between communities, private and not-for-profit sectors and governments. Investments are made in areas with greatest potential for positive population health change. The basic notion is that actions taken earlier in causal streams yield greater population health gains.
